Over the past decade, he has served the Black Hills community with excellence, compassion, and innovation.
He is the first physician in the region to complete a dedicated one-year Endovascular Interventional fellowship at the prestigious Mount Sinai Hospital in New York City. After earning his medical degree in India, he spent four years in New Zealand hospitals as a Medical House Officer and Registrar in Internal Medicine and Cardiology. He then moved to the United States for cardiovascular research, followed by eight rigorous years of clinical training in Internal Medicine, Cardiovascular Medicine, Interventional Cardiology, Vascular/Endovascular Medicine, and Structural Heart Disease at leading U.S. institutions. Dr. Purushottam has performed over 7,000 cardiovascular interventions with an exceptional safety record. He has led pioneering interventions that have saved countless lives and limbs. During his tenure at Monument Health, he and his team achieved a 35% reduction in non-traumatic lower extremity amputations among patients with Chronic Limb Threatening Ischemia. He has also advanced care for cardiogenic shock, launched the region’s laser-assisted IVC filter removal program, built a complex venous disease initiative, and introduced several cutting-edge techniques to the Black Hills. Recognizing the growing burden of cardiovascular disease in the region, Dr. Purushottam founded Midwest Heart and Vascular Associates (MHVA) to enable earlier diagnoses, provide aggressive yet appropriate treatments, and expand access to care. The practice is dedicated to delivering patient-centered, individualized cardiovascular care through state-of-the-art therapies—prioritizing accessibility, timely service, and clear communication at every step. With a streamlined referral process, advanced diagnostics and procedures, a strong emphasis on patient education, and close collaboration with primary care providers and academic institutions when appropriate, MHVA is committed to addressing and overcoming the pressing challenges in cardiovascular care facing the community.
A passionate researcher and educator, Dr. Purushottam serves as Medical Director of Monument Health Research and has led numerous landmark clinical trials that have delivered breakthrough treatments to patients—often before they reach major coastal institutions. He has authored multiple publications and book chapters, presents at national and international conferences, and trains physicians in complex cardiovascular procedures across the U.S. and abroad.He is a co-director of the Peripheral Arterial section at the New York Endovascular Summit, and a frequent invited speaker at LINC in Germany, the world’s largest endovascular conference. He has been invited to lecture and lead training programs in Germany, Italy, Singapore, Malaysia, the UAE, and India. As a Clinical Associate Professor at the University of South Dakota, he actively mentors medical students, residents, and fellows—believing strongly in preparing the next generation of physicians.
Dr. Purushottam is also known for his pioneering contributions, including:
• First U.S. physician to use drug-coated balloon therapy for peripheral arterial disease
• First in the Dakotas to implant the smallest right-sided heart pump (before Mayo Clinic)
• First in the region to successfully treat frostbite using endovascular techniques
• First in the Mountain Midwest to implant a bioabsorbable stent for lower extremity arterial disease
• First to perform a live peripheral arterial intervention at the 2024 Rapid City Cardiovascular Symposium

In his spare time, he enjoys hiking, biking, and skiing in the beautiful Black Hills with his wife, two children, and their dog.
